On those cable channels as designated by Conway Corporation for local ad insertion.
Application:
This rate applies to advertising insertions aired over Conway Corporation cable system.
Terms and rates for local ad insertion on the Conway Cable System shall be based upon market conditions. However, a non-specific-time ad shall not exceed $60.00 for a thirty second insertion and shall not exceed $150.00 for a special event or specific time. A copy of the standard rate card will be available for inspection by the public.
Taxes:
The above rates are subject to all local, state, and federal taxes which are currently in effect or any taxes which are imposed by laws or ordinances on or after the effective date of this rate schedule.
Payment:
The net bill, computed in the accordance with the net monthly rate, shall be due and payable upon presentation.

ESPN is America’s largest cable network, offering the widest and most diverse schedule of sports programming available. 79% of viewers 18 and older watch ESPN weekly.
ESPN2 is original sports programming featuring sports entertainment from the main stream to the extreme. NCAA basketball and football, ESPN2 NFL Tonight, summer and winter X games. (Available for specific events only.)

Application:
This rate applies to leased access channel(s) aired over Conway Corporation cable system.
Taxes:
The listed rates are subject to all local, state and federal taxes which are currently in effect or any taxes which are imposed by laws or ordinances on or after the effective date of this rate schedule.
Payment:
Rates shall be payable in advance for the term of contract. Conway Corporation will allow Lessee to post a letter of credit from an acceptable financial institution for contracts of a minimum of three months. No contract for airtime will be billed in arrears. Contracts for a term are payable no later than the last business day prior to the beginning of the new Term. A lessee who utilizes a letter of credit is advised that payment must be received by the last business day prior to the beginning of a new month or the letter of credit will be called without any additional notice. If there is no Letter of Credit, then Conway Corporation may terminated all time allocated to the Lessee on the first day of the month.

Administrative Fee:
Administrative fees, as applicable, will be added to monthly billing service. A $27 fee, per tape change will be assessed by Conway Corporation cable television system. Such fees will be considered due and payable in the billing period incurred. The $27.00 fee will be waived for non-profit corporations who utilize no more than five (5) tapes in any one-month period. The fee for digitizing tapes will be $40.00 per hour. The fee for technical assistance is $50.00 per hour. An equipment charge of $0.14 will be added to hourly rate for maintenance and purchase of equipment.
Additional Fees:
Due to individual situation of lessee(s), certain additional charges (space rental fees, etc.) may apply. Specific amounts and payment for such will be determined by lessee and lessor at time of contract.
Letter of Credit:
A Lessee may provide a Letter of Credit to Conway Corporation equal to the payment due during the term of the contract.

LEASED ACCESS RATES & POLICY
Effective Date: May 2006
Letters of Request
Requests for part-time leased access must be specific as to exact time of day, length of contract requested and type of programming. Any generic request such as “for any time available” and without a specific term do not meet federal regulatory guidelines. Only requests in writing will be considered valid. Any requests that do not include the three-part requirement will not be considered and will be returned to the Party making the requests. Valid requests will be maintained for six months.
Reselling Time
The Lessee will not be allowed to resell time for more than the price paid by Lessee to Conway Corporation. A violation of this provision will be considered a breach of contract, and Conway Corporation may choose to void our contract relationship with the Lessee.
Right to First Refusal
There shall be no right to continued use of the reserved time on our part-time leased access channels, currently channels 21 and 22. Criteria for contract renewals will be based on, among other things, timeliness of fee and expense payments.
Hourly Rates
9 a.m. to 6 p.m., Monday through Sunday $8.75
6 p.m. to 10 p.m., Monday through Sunday $11.40
10 p.m. to 9 a.m., Monday through Sunday $ 1.20
Daily Rates:
Monday through Sunday $137.59
Contract Fees
The initial contract fee will be $125.00. The renewal contract fee or any amendment to add time to a contract will be $50.00, payable in advance.
Administrative Fees
Administrative fees will be added to your monthly billing service. Fee for digitizing tapes will be $40 per tape or hour. In order to promote diversity in programming, this fee will be waived for non-profit Lessees who utilize no more than five (5) tapes per month. A technical support fee of $50 per hour will be charged for technical assistance provided by Conway Corporation staff but only if we advise you in advance of this charge. This amount will be calculated annually when rates are reviewed.

Insurance Requirements
Lessee shall obtain and maintain in force comprehensive general liability insurance, including personal injury insurance, in the amount of $1,000,000, to insure Lessee and Lessor against any liability resulting from the Service, Lessee’s activities, wrongful acts, errors or omissions in connection with Lessee’s use of the Channel pursuant to this Agreement and for all other risks reasonably associated with the parties’ obligations under this Agreement including, without limitation, full protection to Lessor from any consequences resulting from damage to Lessor’s equipment caused by the attachment, utilization or malfunctioning of any equipment or devices supplied by Lessee or any violation by Lessee of any applicable federal, state or local governmental rule, regulation or ordinance.
